Threads tighten the tissue you have and last months to a couple of years. A facelift repositions the layer beneath and removes skin. The choice is decided by how much loose skin there is, not by budget or downtime.

Facelift or Thread Lift: Which One Do You Actually Need?

Almost everyone asks about threads first, because the downtime is measured in days rather than weeks. The honest framing is that they are not competing options for the same problem. They address different stages of the same process, and choosing the wrong one wastes both the money and the time.

The single question that decides it

Is there excess skin, or only descent?

A thread lift repositions and tightens tissue that is already there. It removes nothing. If the underlying issue is that skin has stretched and there is now more of it than the face needs, threads will lift it briefly and it will settle back down, because the excess is still present.

A facelift removes that excess and repositions the SMAS, the fibrous layer that actually carries facial sag. That is why it lasts years rather than months, and why it costs two weeks of your life.

Pinch the skin along the jawline. If it lifts and there is visible slack left over, threads are the wrong tool.

The literature on threads focuses on technique and material selection rather than on longevity claims, which is itself informative. Polydioxanone threads are the most common material in Korean practice, and published work on pre- and post-procedural considerations treats them as an adjunct in facial rejuvenation rather than as a replacement for surgery.

Where threads genuinely earn their place

For mild early laxity in someone in their thirties or forties, threads do something surgery cannot: they produce a modest, reversible improvement without a scar or a recovery. That is a real category of patient and threads are the right answer for them.

They also work well in combination. Published work describes combining thread-based midface lifting with lower blepharoplasty, where the threads address descent while the eyelid surgery handles the skin. Combination is where thread work is most defensible.

Where threads are oversold

Threads are frequently recommended to patients whose problem is established skin excess, on the reasoning that they are less invasive. The result is a lift that looks good for two months, fades by month eight, and leaves the patient having paid for a temporary version of what they needed.

Two markers that you are being oversold: the clinic proposes threads without examining skin laxity by hand, and the recommendation does not change when you say you would accept surgery.

The Korean market specifics

Thread lifting is inexpensive and widely available in Korea, which is exactly why the recommendation defaults toward it. Thread count and material vary enormously between clinics and are the main driver of both price and result, so a quote without a stated count and material tells you nothing.

Surgical facelift is a different market. It is performed at far fewer clinics, requires general anaesthesia and an operating room, and is a reasonable thing to ask annual case volume about. If a clinic does both and pushes threads hard, ask directly how many facelifts they perform per year. The answer is often the explanation.

If you are travelling for this

Threads are a same-day procedure with three to seven days of visible recovery, which fits inside a short trip. A facelift needs fourteen days in Korea: sutures at seven to ten days, then a second review before flying.

Do not plan a thread lift as a trial run for a facelift. They do not preview the same result, and thread material in the tissue can complicate later surgical dissection in the same plane.

Can a thread lift replace a facelift?

No. Threads tighten tissue already present and last months to about two years. A facelift removes excess skin and repositions the SMAS. Once there is genuine skin excess, threads produce a brief improvement rather than a lasting one.

How long does a thread lift last?

Commonly quoted at one to two years, with the visible peak in the first few months. Duration varies with thread type, count, placement and tissue quality.

Will threads make a later facelift harder?

Thread material in the tissue can complicate dissection in the same plane, particularly if placed repeatedly. Mention any previous thread work at surgical consultation, including dates and thread type.

Are threads and HIFU the same thing?

No. HIFU devices such as Ultherapy deliver focused ultrasound energy to tighten the SMAS layer without inserting anything. Threads are physically placed under the skin to suspend tissue. They are sometimes combined.

What age is right for each?

Age is a poor guide. Skin laxity is the guide. People in their forties with good elasticity and mild descent do well with threads; people of the same age with established jowls do not.
